Desk-job neck pain: strengthening exercise is what the trials support
A 2024 meta-analysis of eight trials in office workers with chronic neck pain found strengthening exercise significantly reduced both pain intensity and disability — though every included trial carried a high risk of bias, so certainty is low.
Chronic neck pain is close to an occupational signature of desk work — long hours of static posture, screens and deadlines. This 2024 systematic review asked what exercise actually does for office workers who already have chronic neck pain, pooling eight randomised trials.
The direction of the evidence was consistent: seven of the eight trials found significant reductions in neck pain, five found reduced disability, and the pooled analysis confirmed that strengthening exercise — targeted work for the neck, shoulder and upper-back muscles — significantly improves both pain intensity and disability. This lines up with the broader neck-pain literature, where strengthening repeatedly outperforms passive approaches for lasting change.
The honesty requirement: every one of the eight trials carried a high risk of bias, so the review grades its certainty as low, and quality-of-life effects went almost unmeasured. The signal is consistent but the science is not yet airtight.
Practically, the combination that makes sense for desk workers is the one we build: a strengthening program for the neck and shoulder girdle as the foundation, ergonomic adjustment of the workstation to reduce the daily provocation, and hands-on treatment for symptom relief while capacity builds. Extended health plans typically cover all three pieces.
