Chronic Whiplash After a Car Accident: What a Major Trial Learned About Exercise and Advice
In a rigorous Lancet trial of people with chronic whiplash-associated disorder, a 20-session supervised exercise program and a single session of professional advice with phone support led to similar improvements — showing that more treatment is not automatically better, and that guided self-management has real value.
Whiplash-associated disorders are among the most common lasting problems after a motor vehicle accident, and for a significant number of people, neck pain and reduced function persist for months. A key question for rehabilitation is how much formal treatment those people need. The PROMISE trial, published in The Lancet, was designed to answer it directly.
Researchers in Australia enrolled adults who had grade 1 or 2 whiplash-associated disorder — neck pain with or without reduced movement and tenderness, but without major neurological injury — lasting longer than three months. Participants were randomly assigned to one of two approaches: a comprehensive, individually tailored exercise program of 20 supervised sessions over 12 weeks, or a single session of professional advice about staying active and managing symptoms, backed up by telephone support.
The result surprised many clinicians. Both groups improved over time, and the intensive 20-session program did not produce meaningfully better pain outcomes than the advice-based approach. In other words, for this chronic whiplash population, a large dose of supervised treatment was not automatically superior to well-delivered guidance and encouragement to stay active.
It is worth being careful about what this does and does not mean. The trial does not say exercise is useless for whiplash — participants in both groups were encouraged toward activity and both improved. Rather, it shows that treatment intensity should be tailored: some people do well with expert assessment, a clear plan, and support to self-manage, while others may need more hands-on supervision to progress safely and confidently. Honest, evidence-based rehabilitation means giving each person the level of care their situation actually requires — and empowering them with the knowledge and confidence to keep moving on their own.
