Exercise prevents falls: 23% fewer, across 23,000 older adults

One of Cochrane's landmark reviews: across 108 trials and 23,407 community-dwelling older adults, exercise programs cut the rate of falls by 23% — roughly 195 falls prevented per 1,000 people each year — with balance and functional training working best.

For older adults, a fall is rarely just a fall — it is the event that most reliably converts independence into decline. Hip fractures, hospital stays, and the fear that shrinks activity all radiate from it. This Cochrane review, one of the largest in rehabilitation, asked whether exercise genuinely prevents falls in older people living at home.

The answer, across 108 randomised trials and 23,407 participants averaging 76 years old, was an unambiguous yes. Exercise programs reduced the rate of falls by 23% — in absolute terms, about 195 falls prevented for every 1,000 people per year. The most effective programs trained balance and functional movement — standing, stepping, reaching challenges — either alone or combined with resistance training. Walking programs and general activity alone did not show the same protective effect: the balance challenge appears to be the active ingredient.

The caveats are typical of a field this size: many trials had unclear or high risk of bias, adverse events were under-reported, and measured quality-of-life gains were marginal. None of that disturbs the central finding, which is consistent, dose-sensible and biologically coherent.

The practical message for families: falls are not an inevitability of age, they are partly a trainable skill deficit. A properly built program challenges balance safely and progressively — which is precisely the kind of supervised programming a kinesiologist provides, in the clinic or in community settings.
