Fear, confidence and motivation: the psychological side of returning to sport after injury
This systematic review found that whether an athlete returns to sport after injury depends as much on psychological readiness — confidence, motivation and fear of reinjury — as on physical recovery.
An athlete can pass every strength and movement test and still not return to their sport. This systematic review set out to explain why, gathering the published evidence on psychological factors associated with returning to sport after an athletic injury.
Across the included studies, a consistent picture emerged. Athletes who returned to their pre-injury level of competition showed more positive psychological responses: stronger motivation through rehabilitation, greater confidence in the injured body part, and less fear of getting hurt again. Fear of reinjury stood out in the other direction — it was among the most commonly reported reasons athletes gave for stepping away from sport, even when their physical recovery was considered adequate.
The authors framed the findings using self-determination theory, which emphasises autonomy, competence and relatedness as drivers of motivation. Rehabilitation that rebuilds a sense of competence — graded exposure to sport-specific demands, measurable milestones, and involvement in decision-making — maps directly onto those drivers.
The review is limited by the observational nature of most included studies: psychological readiness and successful return are associated, but the studies cannot fully prove one causes the other. Later work by the same group developed dedicated scales for measuring psychological readiness before return to sport.

Clinical relevance
Clearing tissue-healing and strength benchmarks is necessary but not sufficient. A structured, progressive return-to-sport pathway — where the athlete experiences competence at each stage rather than being sent back on a date — addresses the confidence and fear factors this review identified.
