Massage after surgery: measurable relief for post-operative pain
Pooling 10 randomised trials with 1,157 surgical patients, this meta-analysis found massage therapy meaningfully improved post-operative pain (SMD −0.49) with unusually consistent results across trials — while anxiety findings were more mixed.
Recovery from surgery is dominated by two experiences: pain and worry. Medication handles the first up to a point, but it comes with limits and side effects, which is why hospitals have studied massage as an adjunct. This meta-analysis pooled ten randomised trials — 1,157 patients — testing massage therapy after operations.
The pain result was notably clean. Massage produced a moderate improvement in post-operative pain, and the trials agreed with each other to an unusual degree (heterogeneity of just 4% — rare in massage research). Effects were largest in patients recovering from sternal incisions after cardiac surgery, a famously uncomfortable recovery.
The caveats: overall methodological quality of the included trials was low, which limits how confidently the numbers generalise, and the anxiety results — while promising in several trials — were too inconsistent to pool cleanly. Massage after surgery is an adjunct; it never replaces surgical follow-up care, wound management or the rehabilitation plan.
Used inside that plan, though, the logic is straightforward: less pain means earlier movement, better sleep and less reliance on medication — each of which feeds the active recovery that determines the final outcome. That is how we deploy orthopaedic massage alongside post-operative physiotherapy and kinesiology.
